Get to know the Nurses at The Park Medical Practice...
The Nurses are multi-skilled with individuals having specialist areas of expertise. Practice nurses and District Nurses work as one team providing appropriate care in the home or surgery.
If you require advice on any illness or a health related matter you can ask to speak to a nurse if one is free, if not we will be happy to take down a contact number and call you back.
Practice nurses can be seen by making an appointment through reception.
The District Nurses can be contacted through the surgery or on 366717 if urgent.
Nurse Practitioner Jayne Horwich
Qualifications BA Hon's, RN Specialist Practitioner (Practice Nursing), Certificate in Health Promotion, NVQ Assessor, Marie Curie Certificate. ENB 501 Family Planning, Nurse Prescriber (Extended and Supplementary, Nurse Prescribing (independent) Supplementary and Extended PG Certificate in Primary Care Studies.
Nurse Practitioner Horwich has worked at the Practice since 1987, initially as a Practice Nurse, but for the last 2.1/2 years as a Nurse Practitioner who sees patients with minor illness and chronic conditions, being able to prescribe for a range of conditions.
Nurse Practitioner Lois Reeves
Qualifications RGN, Independent Prescriber.
Nurse Practitioner Reeves is experienced in sexual health, family planning and working with young people. She has been with the practice for 2 years and thoroughly enjoys her job and the people she meets.
Practice Nurse Team Leader Julie Lowe
Qualifications RGN
Julie is our Practice Nurse Team Leader for Chaddesden Surgery and also our University site.
Nurse Sally Rigg
Qualifications RN – Courses on Diabetes, Chronic Obstructive Pulmanary Disease, Travel Health, Cervical Screening.
Sally performs general practice nurse duties and has a particular interest in Diabetes, COPD and travel health.
Nurse Sabina Attfield
Qualifications RGN, Registered Nurse Child Branch, Diploma in Nursing.
Over the last 17 years Sabina has worked in an hospital environment in Adult Medicine, Surgery and Orthopaedics and also in Paediatric Trauma and Medicine. She commenced employment with The Park Medical Practice in 2005 and currently works predominantly at the University branch of the practice.
Community Staff Nurse Lorna Turner
Qualifications Dip.N. (Adult) RN
Lorna works within the district nursing team providing care to housebound patients. She has interests in Palliative Care.
District Nursing Sister Zena Sharp
Qualifications Dip. Nursing (Adult), BSc (Hons) Specialist Community Practice.
Zena leads a team of community nurses and provides care for housebound patients. She has a particular interest in palliative care and wound management.
Community Staff Nurse Leanne Cooney
Leanne is a District Phlebotomist and Auxiliary Nurse having been with the practice for 2.1/2 years. She enjoys knowing that she has made a difference to patients needs.
Community Staff Nurse Susanna Wilsoncroft
Qualifications Dip.N.(Adult) RN.
Susanna works within the district nursing team providing care to housebound patients. She has interests in Palliative Care, Wound Management and Care of the Elderly.
